We are looking forward to another great fall sports season. As you make plans for the upcoming school year, WCA will offer the following sports this fall:

REGISTRATION
To register for a fall sport and participate in practices, submit the following items to the school office before the first practice:

1. Athlete Permission & Commitment Form 2018-19
2. Updated IHSA Physical Form.

SPORTS FEE must be paid in full before athletes will be issued their uniform.

SUBMIT TO SCHOOL PRIOR TO THE FIRST DAY

Illinois Medical Exam Requirements: Consult your physician for details.

Child Health Exam Form (including immunization records) are required for:

• • New students

• Kindergarten / 6^thgrade* / 9^th grade students

Tdap Vaccine: Students entering 6^th through 12^th grades are required to show proof of receipt of one dose of Tdap vaccine.

MCV4 vaccines: ALL 12^th grade students must show proof of receiving two MCV4 vaccines (meningococcal conjugate).

Dental Exams are required for:

• • Kindergarten

• • 2^ndgrade

• 6^th grade students

Vision Exams are required for:

• • Kindergarten students

• Students transferring from out of state.

Sports Physicals are required for ALL students participating in the athletic program prior to starting practice.

Forms may be obtained from your health care provider or the school office or under Documents.

Parents, if you plan to allow your child to receive over the counter or prescription medication at school, submit the following form: Request for Medication 2018-19.
