Please take a moment to complete this form and let us know which days and times your child will be attending our Pre-K (4yr old) class for 2019–20. We are already receiving new applications for Pre-K, and we would like to reserve your child's place for the days of your choice. If your family chooses not to attend Westlake for Pre-K, please indicate this on the next page. You must "opt out" by March 31 to avoid being charged a fee.
